📋 Description

• Provide integrated pharmacotherapy support to patients and physicians.
• Deliver remote assessments and hybrid onsite collaboration for therapy management services.

🎯 Requirements

• PharmD and CA pharmacist licensure; advanced practice pharmacist in CA (per CA criteria).
• Strong knowledge of pharmacology, FDA guidelines, and therapy management.
• Excellent communication, documentation, and collaboration skills.
• Proficiency with Microsoft Office and data management; ability to work remotely and with hybrid
