📋 Description

• Telehealth-enabled care coordinator in an Advanced Care at Home program.
• Supports patient throughput, care coordination, and longitudinal case management across the

• Coordinates day-to-day care activities for a virtually managed patient population.
• Collaborates to ensure timely, appropriate, and coordinated services per clinical plans and program

🎯 Requirements

• Active LVN/LPN license in good standing. CA License preferred.
• 2+ years clinical experience (acute, home health, ambulatory, or case management).
• Experience with EMR/EHR systems and clinical workflows.
• Strong telephonic/virtual communication skills.
• Ability to manage multiple priorities in a fast-paced, remote environment.
• Demonstrated ability to work within scope and escalate appropriately.
