Telehealth-enabled care coordinator within Advanced Care at Home
Supports patient throughput, care coordination, and longitudinal case management
Operate under RN/provider supervision and within licensure scope
Coordinate day-to-day care activities across a virtual patient population
Escalate appropriately when needed

🎯 Requirements

Active LVN/LPN license in good standing. CA license preferred
2+ years clinical experience (acute/home health/case management)
Experience with EMR/EHR systems and clinical workflows
Strong telephonic/virtual communication skills
Ability to manage multiple priorities in remote setting
Demonstrated ability to work within scope and escalate
