Care Coordinator, RN (MSHO)

📋 Description

• Hybrid position traveling locally to meet with members in homes and assisted living facilities
• Manage DHS and CMS requirements for MSHO/MSC+ members
• Support access to medical care, waiver services, and community resources
• Home-based visits with care coordination for seniors in Minnesota, Wisconsin, and Florida

🎯 Requirements

• Minnesota Registered Nurse license or MN Licensed Social Worker (LSW, LMSW, LISW, or LICSW) - new
• One or more years of geriatric or certified disabled population case management/care coordination
• Must have a valid driver’s license
• Ability to work independently and in a private home office
• Strong communication, interpersonal, and customer service skills
• Read, write, speak, and understand English proficiently
