Care Manager - 1099 Contract $50 per visit (Hartford, CT / Field-based)

📋 Description

• Contract Care Manager position in Connecticut (field-based).
• Provide orientations, assess new patients, and train caregivers for in-home care services.
• Perform home care assessments, caregiver fitness checks, and in-home orientations with paperwork.
• Travel within the state for patient coverage and ensure high-quality care delivery.

🎯 Requirements

• 2+ years experience with frail or elderly disabled adults and caregivers (preferred).
• Field experience in a medical or case management environment (preferred).
• Experience with data input, case review, or Salesforce is a plus; strong MS Office skills (Word
• Valid driver’s license with insured vehicle for travel within the state.
• Excellent verbal/written communication and organizational skills; strong problem-solving abilities.
• Hands-on home care experience and knowledge of ADL/IADL assessments preferred.
