Care Manager (Bloomington, IN / Field-based)

📋 Description

• Orient new patients to the program and assess home care needs
• Train caregivers to deliver quality in-home care
• Coordinate and communicate with patients, caregivers, and staff
• Perform initial assessments and provide ongoing supervisory visits
• Travel within the greater Bloomington, IN area to support cases

🎯 Requirements

• 2+ years of experience with frail or elderly disabled adults and caregivers
• Field experience in a medical or case management environment, preferred
• Hands-on experience in home care, preferred
• Working knowledge of ADL/IADL assessments
• Valid driver’s license with an insured vehicle for travel within the state
• Excellent verbal and written communication, organizational and time management skills
