Care Manager (Gordon & Whitfield, GA / Field-based)

📋 Description

• Perform orientations for new patients in the FreedomCare program.
• Assess patients to determine care needs and training requirements for caregivers.
• Train caregivers to ensure high-quality, compassionate home care.
• Coordinate field-based care across Gordon & Whitfield, GA; travel within GA as needed.
• Engage with interdisciplinary teams, caregivers, and patients to ensure effective care.
• Support patients and families to promote independence and quality of life.

🎯 Requirements

• 2+ years of experience working with frail or elderly disabled adults and caregivers
• Excellent verbal and written communication, organizational and time management skills
• Hands-on experience in home care; valid driver’s license with insured vehicle for travel
• Proficient with Microsoft Office (Word, Excel, Outlook); Salesforce experience a plus
• Strong critical thinking, problem-solving, and team-player mindset
• Willingness to travel within Georgia; ability to engage patients and caregivers effectively
