📋 Description

• Conduct initial in-home clinical assessments for patients in Ohio.
• Orient patients and caregivers to care plans and services.
• Provide ongoing care management and coordination with PCAs and interdisciplinary teams.
• Visit patients regularly to monitor well-being and safety and ensure regulatory compliance.
• Coordinate documentation, communication with physicians, and activation of services.

🎯 Requirements

• Registered Nursing license in Ohio (Active)
• Care coordination and in-home case management experience
• Strong communication, organization, and documentation skills
• Proficiency with Case Management Systems and MS Office
• Ability to provide patient- and family-centered education
• Willingness to travel within assigned geographic area
