  • Coordinate documentation, communication with physicians, and activation of services.

  • Registered Nursing license in Ohio (Active)
  • Care coordination and in-home case management experience
  • Strong communication, organization, and documentation skills
  • Proficiency with Case Management Systems and MS Office
  • Ability to provide patient- and family-centered education
  • Willingness to travel within assigned geographic area
