Care Manager, RN (Lorain OH / PART-TIME / Field-based)

📋 Description

• Conduct initial in-home assessments and orientations for patients and caregivers.
• Provide ongoing care management and coordination with interdisciplinary teams.
• Educate patients, PCAs, and families; ensure quality assurance and regulatory compliance.
• Coordinate service activation, care plans, and ongoing monitoring through home visits.
• Maintain documentation and communicate clinical concerns for timely intervention.
• Ensure continuity of care and adherence to agency policies.

🎯 Requirements

• Registered Nurse (RN) with valid license in Ohio.
• Care coordination and in-home case management experience preferred.
• Proficiency with MS Office and Case Management Systems; ability to teach others.
• Strong communication, empathy, and patient-centered approach.
• Ability to travel within assigned geographic area; valid driver’s license and car insurance.
• Diploma from an accredited nursing program; new graduates welcome.
