Care Manager, RN (Portland, OR / Hybrid / Per Diem)

📋 Description

• Conduct initial in-home assessments and orientations for new patients and caregivers
• Provide ongoing care management and education to patients and PCAs
• Coordinate with patients, PCAs, and interdisciplinary teams for care plans
• Schedule regular home visits; monitor wellbeing and care quality
• Identify barriers to care and ensure timely intervention and regulatory compliance
• Communicate clinical concerns to the care team to optimize outcomes

🎯 Requirements

• Diploma from an accredited nursing program
• Valid and Active Registered Nursing License issued by the Oregon Board of Nursing
• Proficiency with case management processes and standard documentation
• Ability to coordinate care and educate patients and caregivers
• Strong communication and collaboration with interdisciplinary teams
• Preferred: Bilingual capabilities
