Case Manager I (Medications for Opiod Use Disorder)

📋 Description

• Outreach and link Glide HEAT clients to MOUD (methadone, suboxone, naltrexone, etc.) at Glide and
• Build positive clinical rapport to promote adherence and reduce harm.
• Refer clients to primary care, mental health services, housing and necessary documentation.
• Maintain a productive caseload with MOUD clients to promote stabilization.
• Coordinate engagement in Glide programs and HEAT services through community outreach.
• Support client housing, identification and access to supportive resources.

🎯 Requirements

• Bachelor’s degree in social work, public health or related field OR 2+ years MOUD navigation and
• Experience and training in overdose reversals and de-escalation techniques.
• Experience with street-based outreach in urban settings and homeless encampments.
• Knowledge of social determinants of health and serving diverse populations.
• Strong communication and relationship-building skills; ability to educate and engage clients and
• Must understand HIPAA protocols and maintain professional boundaries.
