Case Manager, RN

📋 Description

• Guide participant care on the IDT in PACE
• Assess participant needs and develop care plans
• Coordinate transitions of care and follow up
• Deliver and document nursing interventions
• Maintain safe environments and timely physician orders
• Communicate with participants and families about plans

🎯 Requirements

• RN licensed in California (unencumbered)
• BSN preferred
• BLS/First Aid current
• CPR/First Aid within 180 days if not current upon hire
• 2 years nursing experience with frail/elderly population preferred
• Excellent communication and care coordination skills
