Case Manager, RN (Douglasville/Atlanta, GA)

📋 Description

• RN in home care; perform clinical evaluations in members’ homes.
• Conduct monthly visits to ensure quality service and address condition changes.
• Develop and adjust service plans with the Adult Foster Care program.
• Coordinate care with physicians, RNs, and other disciplines; document visits.
• Provide caregiver and member support; follow agency policies and procedures.
• Travel within the state as needed; maintain patient privacy and compliance.

🎯 Requirements

• Registered Nurse licensed by Georgia Board of Nursing.
• Minimum 2 years clinical experience; 1 year with elders or adults with disabilities.
• Bilingual in Spanish is a plus; CRM experience (Salesforce a plus).
• Valid driver’s license with an insured vehicle for travel within the state.
• Strong communication, organization, and problem-solving skills.
• Ability to thrive in a fast-paced startup environment.
