Case Manager SUDP - Community Mental Health

📋 Description

• Provide community-based care coordination and recovery support for individuals with mental health
• Serve as primary support and case manager for clients with mental health conditions and SUD.
• Coordinate services with healthcare providers and community resources to support independence.
• Assist clients in developing recovery goals and person-centered care plans.
• Document case notes and maintain service records per program standards.
• Collaborate with multidisciplinary teams to deliver comprehensive behavioral health care.

🎯 Requirements

• High school diploma or GED required.
• Active Washington State SUDP credential required.
• Minimum six months experience in addictions treatment or substance use setting.
• Strong interpersonal skills; ability to build trust with clients.
• Ability to work with a multidisciplinary care team.
• Commitment to culturally responsive, person-centered services.
