  • Collaborate with interdisciplinary care team to reduce avoidable healthcare utilization.
  • Support ambulatory care services and quality initiatives as needed.
  • Participate in program development, quality metrics, and reporting of outcomes.

🎯 Requirements

  • Providence or RI licensure required; unrestricted Rhode Island pharmacist license.
  • Board certifications or ASHP residency in ambulatory care preferred; PharmD degree required.
  • Experience with COPD/respiratory care, inhaler technique education, and population health.
  • Experience under a Collaborative Practice Agreement (CPA) and independent medication management.
  • Strong documentation, EHR usage, and interprofessional collaboration skills.
  • Knowledge of chronic disease management, transitions of care, and quality improvement.
