Motivate behavioral changes through education, peer support, and shared experiences.
Coordinate with clinical teams to develop and carry out care plans.
Document information from encounters in designated information systems.
Assist enrollment into Akido ECM Program and connect to services.
Travel 50-75%; requires valid driver's license and reliable transport.

🎯 Requirements

High School Diploma or equivalent.
Bilingual in English and Spanish strongly preferred.
2+ years experience with CalAIM populations (homelessness, SMI/SUD, post-incarceration, complex
Ability to work outdoors, independently and in teams, manage competing tasks.
Excellent communication and interpersonal skills; proficient in documenting in case management
Compensation: hourly, with pay range $22-$25 USD.
