Collaborate with Care Coordinators in scheduling home visits for efficiency

🎯 Requirements

1+ years of experience in field-based roles
Capable of assessing patient health risks and identifying hazards in the home
Motivation to meet case management targets
Excellent written and verbal communication skills
Excellent time management, critical thinking, active listening; multitasking
Own reliable transportation and ability to travel daily
