Field Case Manager

📋 Description

• Customize patient support program elements to meet the needs of assigned prescribing physicians
• Review patient information and medical records to support enrollment and completion of public and
• Identify reimbursement barriers and escalate relevant issues to the Program Manager.
• Build strong relationships with physicians and clinic staff, providing ongoing support and training
• Welcome patients into the program, collect required information, explain reimbursement guidelines
• Navigate public and private reimbursement processes, including special authorizations, appeals

🎯 Requirements

• Bachelor’s degree in healthcare or a related discipline, or a nursing diploma.
• Active registration as an RN or RPN/LPN with the applicable provincial College of Nurses is
• Minimum of 5 years of experience in healthcare, pharmaceuticals, insurance, or a related industry.
• At least 3 years of nursing experience in a clinical setting is considered an asset.
• Previous case management experience is preferred.
• Experience with reimbursement navigation, special authorization processes, appeals, and field-based
