Home Care Coordinator (LVN)

📋 Description

• Role: Home Care Coordinator to deliver personal and clinical home care in the IDT setting.
• Assess participant needs, develop care plans, and coordinate home care services.
• Coordinate with clinical and non-clinical team members to support safe, person-centered care at
• Maintain comprehensive documentation and ensure alignment with participant goals.
• Collaborate with caregivers, participants, and family members to identify care gaps and service

🎯 Requirements

• Minimum of two (2) years of clinical experience, preferably in home care or geriatrics.
• Active LVN license in the applicable state.
• Strong communication, care planning, and interdisciplinary collaboration skills.
• Ability to perform comprehensive in-home assessments and coordinate services.
