Home Care Coordinator Supervisor

📋 Description

• Oversee HCC team and collaborate with Home Services and external partners
• Coach and mentor on documentation for compliance and consistency
• Lead pre-enrollment assessments with MOE team; attend weekly meetings
• Manage direct reports: hiring, training, supervising, mentoring
• Investigate conflicts; coordinate with HR for outcomes
• Lead daily HCC meetings on assessments and hospitalizations

🎯 Requirements

• Healthcare or medical licensure plus 3 years of professional exp
• Bachelor’s degree preferred
• 3+ years case management or home nursing exp with frail/elderly
• CPR and First Aid cert within 180 days (preferred at hire)
