Home Care Supervisor

📋 Description

• Oversee the Home Care Assistant team delivering activities of daily living in participants\' homes.
• Manage scheduling oversight and relationships with external contracted vendors.
• Ensure compliance, training, and documentation accuracy with care plans and certifications.
• Coordinate with Home Health Manager and Rehabilitation department on participant home exercise
• Support care quality and participant interactions to meet WelbeHealth standards.
• Contribute to an encouraging, patient-centered care environment.

🎯 Requirements

• High school diploma or equivalency; bachelor’s degree preferred
• Minimum 1 year overseeing Home Care Assistants with frail/elderly populations
• 2 years of supervisory experience with mentoring/developing teams
• Reliable transportation
• CPR and First Aid certification within 180 days of hire; preferred at hire
