Conduct home health nursing assessments in patients' homes
Actively participate in IDT meetings to develop care plans
Deliver and document home health nursing interventions per care plans
Instruct patients, families, and caregivers on disease, self-care and prevention
Track and monitor home health scheduling with the HH Manager
Collaborate with the Physician and IDT for smooth care transitions

🎯 Requirements

Graduate of an accredited nursing program
Unencumbered RN license required
One year nursing exp with frail/elderly; home health exp preferred
Reliable transportation
CPR and First Aid within 180 days of hire (preferred)
