📋 Description

Provide skilled home health nursing under CMS guidelines
Collaborate with IDT to develop and coordinate care plans
Deliver nursing interventions and document medications and tests
Educate participants and families on disease processes and self-care
Coordinate scheduling and smooth care transitions with the team
Maintain a safe home environment and ensure timely physician orders

🎯 Requirements

Graduate of an accredited school of nursing
Unencumbered RN license
Minimum 1 year nursing experience with frail/elderly; home health preferred
Reliable transportation
Must obtain CPR and First Aid within 180 days of hire
CPR/First Aid certification upon hire preferred
