📋 Description

Provide skilled, compassionate home health nursing per CMS guidelines
Coordinate care plans with the interdisciplinary team (IDT)
Deliver and document nursing interventions, maintain safe home environment
Respond to physician orders and administer medications as ordered
Educate participants, families, and caregivers on disease process and self-care
Track home health scheduling with Home Health Manager

🎯 Requirements

Graduate of an accredited school of nursing
Unencumbered RN License
Minimum 1 year nursing experience with frail/elderly population; home health preferred
Reliable transportation
CPR and First Aid within 180 days of hire; preferred at hire
