Provide hands-on personal care and assistance with activities of daily living (ADLs) in
Assist with instrumental ADLs: laundry, light housekeeping, shopping, light meals
Measure and document participants’ vital signs as instructed
Act as medical escort to appointments and support safe transfers
Assist Rehabilitation team with home exercise programs per care plan
Provide translation services as needed

🎯 Requirements

High school diploma or equivalency
Current BLS/First Aid Certification required
CNA or HHA certification preferred
Bilingual preferred
Must be registered with Home Care Registry prior to start date
CPR and First Aid certification within 180 days of hire and maintain certification thereafter
