• In-office role with daily travel to Albany FreedomCare office
• Coordinate patient, caregiver, and nurse coverage for HHA needs
• Pair caregivers and patients for best fit and care plan execution
• Maintain scheduling records and compliance with regulatory requirements

🎯 Requirements

• 2+ years in a similar role
• LHCSA experience preferred; medical exp a plus
• HHA Exchange software experience strongly preferred
• Creative, proactive problem solver; strong process improve
• Excellent customer service; strong written and verbal comms
• Tech proficient; comfortable with computer and phone systems
