Licensed Practical Nurse (LPN)

📋 Description

• Provide quality health care management for individuals with disabilities
• Conduct routine site visits, check vitals, assess, and document progress
• Act as health/medical resource and train staff
• Conduct medication audits and ensure policy compliance
• Support on-call triage for medical concerns as needed

🎯 Requirements

• Current Licensed Practical Nurse (LPN) license in Indiana
• 1-2 years’ experience with disabilities or long-term care preferred
• Strong verbal/written communication and independent work ability
• Organizational and computer skills; background check required
