📋 Description

• Support health care needs in adult residential facilities in Portage, IN
• On-call triage and periodic site visits for assessments
• Document progress and ensure compliance with medical records
• Provide case management support and staff training in medical arena
• Full-time, Mon-Fri with no weekends, occasional travel

🎯 Requirements

• Current Indiana Licensed Practical Nurse (LPN) license
• 1-2 years with disabilities or long-term care preferred
• Strong communication and independent work ability
• Organizational and computer skills; background checks required
