Licensed Social Worker (LSW)

📋 Description

• Oversee patient care within the Structured Family Caregiving (SFC) program in Ohio.
• Provide ongoing support to patients and caregivers; serve as escalation point and advocate for
• Conduct in-person home assessments; ensure plan of care is communicated and compliant.
• Monitor day-to-day patient care and caregiver coaching; support program integrity and issue triage.
• Maintain detailed case records following agency policy and procedures.
• Collaborate with the physician plan of treatment and triage changes as needed.

🎯 Requirements

• Valid and active Ohio Licensed Social Worker license (CSWMFT Board).
• Bachelor’s or Master’s degree in Social Work from CSWE-accredited program.
• Minimum 2 years clinical experience; 1 year with elders or adults with disabilities; home care
• Spanish bilingual a plus; CRM experience (Salesforce a plus); valid driver’s license with insured
