Care for WelbeHealth participants in the clinic setting.
Support clinic operations, monitor vitals, perform point-of-care testing, and administer
Coordinate with providers on medication orders and changes from procedures.
Review medications during Welbe Welcome Day and follow post-procedure instructions.
Escalate questions to RN Case Manager or PCP as needed.

🎯 Requirements

Graduate of an Accredited School of Nursing with an unencumbered LVN/LPN license.
Minimum one year nursing experience with frail/elderly populations.
Strong clinical, organizational, and communication skills with seniors and interdisciplinary teams.
CPR and First Aid certification within 180 days of hire (preferred at start).
