📋 Description

• Provide clinical assistance as part of care teams traveling to senior living communities
• Perform pre-visit planning, chart prep, and patient screenings
• Reconcile medications and maintain accurate patient charts
• Assist with minor procedures under provider supervision
• Manage portal messages and coordinate with providers and staff
• Deliver care that is preventative, proactive, and tailored to patients' needs

🎯 Requirements

• Active LPN license or CMA/RMA/CCMA certification
• Valid driver’s license required
• Experience with electronic medical records and medication reconciliation
• Strong interpersonal and communication skills
• Ability to work in field settings and manage clinical flow
• English language proficiency
