📋 Description

• MSW to support elderly participants within PACE program
• Role includes assessments, care plans, and coordination with IDT
• Provide discharge planning and care transitions support
• Collaborate with primary care and care team for care coordination
• Deliver social work interventions per care plan, including counseling and case management
• Maintain compliant documentation and participate in care conferences

🎯 Requirements

• Master’s degree in Social Work (MSW)
• Minimum 1 year experience with frail/elderly population
• Experience coordinating and facilitating care conference meetings
• Proof of medical clearance for communicable diseases (TB test)
• Proof of up-to-date immunizations; CPR/BLS certification or willingness to obtain within 30 days
• Valid CA driver’s license, transportation, good driving record, auto insurance
