Collaborative Care Clinician I provides care for pediatric and/or adult patients.
Uses evidence-based interventions to reduce symptoms.
Works with Care Pod to customize patient treatment plans.
Collaborates with PCPs and other providers for integrated care.

🎯 Requirements

LMSW or LCSW licensed in North Carolina
Master’s Degree in Social Work or related field
1-5 years licensed clinician experience
Fluent in English and Spanish (medical interpretation)
Proficient with EHRs and Salesforce experience preferred
