Nurse Practitioner (Remote, NJ License Required)

📋 Description

• Cover care needs for Transitions of Care program
• Telephonic outreach and scheduled video/phone visits after hospital discharge
• Medication reconciliation and follow-up lab/medication ordering
• Explain Galileo care model and connect patients to services
• Coordinate care across hospital, ED, pharmacies, and community providers
• Collaborate with nursing team and contribute to population health goals

🎯 Requirements

• Certified Nurse Practitioner in Adult, Family or Geriatrics
• Active NJ state license; ability to obtain additional state licenses
• 5+ years in Primary Care or Geriatrics; 2 years may be in Skilled-Nursing/LTC
• Experience with Medicare/Medicaid populations with complex conditions
• Motivational interviewing skills for behavioral changes
• Comfort with digital-first/virtual care; solid tech with Google Suite and Slack a plus
