🎯 Requirements

• Certified Nurse Practitioner in Adult, Family or Geriatrics with active PA state licensure.
• 4+ years clinical experience in Primary Care or Geriatrics preferred; 2+ years may be from
• Experience with Medicare/Medicaid populations and complex comorbid conditions.
• Motivational interviewing, strong communication, and comfort with digital/virtual care delivery.
• Tech proficiency; Google Suite and Slack a plus; bilingual English/Spanish a bonus.
