Occupational Therapist

📋 Description

• OT evaluation and treatment for PACE participants
• Develop individualized treatment plans
• Conduct home visits as needed
• Instruct participants, caregivers, staff on ADLs, exercise, and DME
• Collaborate as core IDT member for care planning
• Document to regulatory standards

🎯 Requirements

• Masters/Doctorate in OT (MOT or DOT) or equivalent
• National Certification by AOTA or eligible for exam
• At least 1 year with frail or elderly
• Spanish/English bilingual strongly preferred
• PACE experience or ADCH/CBAS a plus
• State driver’s license and reliable transportation
