Patient Engagement Specialist

📋 Description

• Support patients and clinical care team members with engagement efforts.
• Ensure patients achieve initial activation and ongoing engagement with Evergreen Nephrology.
• Collaborate with market operations teams across the US to engage patients with their care teams.
• Work remotely across time zones, with preferred CST hours.

🎯 Requirements

• High School diploma or GED; Bachelor’s Degree preferred
• 3+ years in patient support engagement or healthcare customer service
• Strong verbal/written communication; sales aptitude a plus
• Experience in a team-based service environment; multitasking to meet metrics
• Medicare or kidney care experience a plus; bilingual Spanish/English a plus
• Proficient with MS Office (Outlook, Teams)
