Physical Therapist

📋 Description

• The Physical Therapist provides evaluation and treatment across acute, restorative, and maintenance
• Develops individualized treatment plans and conducts home visits as needed.
• Educates participants and caregivers on exercise programs and DME use.
• Contributes to the Interdisciplinary Team (IDT) with assessments and care coordination.
• Maintains documentation meeting regulatory requirements and supports program standardization.

🎯 Requirements

• Bachelor’s Degree in physical therapy
• Active California Physical Therapist license
• Current CPR/BLS certification
• Excellent communication with participants and caregivers; documentation and care coordination skills
• Experience with documentation, regulatory compliance, and safety protocols
• Preferred: 2+ years as a practicing PT; bilingual Spanish/English strongly preferred
