Physical Therapist - Part Time

📋 Description

• Part-time Physical Therapist providing evaluation and treatment across care levels for PACE
• Develop individualized treatment plans; conduct home visits as needed.
• Educate participants and caregivers on exercise programs and DME use.
• Contribute to IDT care planning, maintain regulatory documentation.
• Support PT program standardization, team education, and quality audits.

🎯 Requirements

• Bachelor’s Degree in physical therapy or higher (DPT preferred); current California PT license.
• Completion of an APTA-accredited PT program.
• Active CPR/BLS certification; ability to perform home visits and DME guidance.
• 2+ years of PT experience in geriatrics or with frail/elderly populations preferred.
• Strong communication for caregiver education and interdisciplinary collaboration.
