📋 Description

• Support high-acuity and medically complex populations in care and recovery journeys.
• Combine population health management, care coordination, data analysis, and direct member
• Focus on pregnant/postpartum members, recently hospitalized individuals, and those with complex
• Partner with medical providers, counselors, specialists, and care teams across regions within a
• Identify care gaps, monitor member progress, coordinate interventions, and contribute to quality
• Contribute to pilot programs and improve care delivery for vulnerable populations.

🎯 Requirements

• Current Registered Nurse (RN) license; Maine LPN with SAMHSA X-DEA training may be eligible.
• Bachelor's degree in Nursing preferred.
• At least 5 years of direct patient-care experience in addiction medicine, behavioral health, or
• Strong care coordination, population health, and interdisciplinary delivery knowledge.
• Understanding of HIPAA and healthcare regulations, professional conduct standards.
• Clear communication with dispersed teams and diverse member populations.
