Primary Care Physician

📋 Description

• Act as a key PCP for our patient population, delivering care in the clinic and digitally
• Provide primary care across the full scope of preventive health, chronic disease management, urgent
• Collaborate and partner across care team members including behavioral health, virtual urgent care
• Educate patients on their health conditions and develop individualized care and treatment plans
• Solve patient problems in an efficient, nimble manner by drawing on resourcefulness, collaborating

🎯 Requirements

• 5+ years of clinical experience in chronic care and primary care for all ages
• Comfort with basic procedural skills
• Active board certification in Family Medicine with comfort seeing Pediatric patients
• Active licensure in Texas or ability to obtain license by start date with Galileo
• Strong communication skills, especially the ability to translate complex ideas into simple language
• Bi-lingual in English and Spanish is a plus
