📋 Description

• Oversee PACE functions: hiring, training, supervising, mentoring staff
• Develop and facilitate IDT and participant-care meetings
• Oversee SOPs and regulatory adhearance across teams
• Collaborate on participant services: home care, home health, transport
• Manage site, supplies, equipment, and vendor oversight

🎯 Requirements

• Bachelor's in health admin/health care mgmt/gerontology or related; master’s preferred
• Experience in nursing, social work, or rehabilitative therapy with frail elderly
• Seven+ years in senior health program mgmt; 3+ in supervision
• Multi-disciplinary mgmt experience; value-based care preferred
• CPR and First Aid certification within 180 days (or on hire)
