Rehabilitation Service Specialist (RSS)

📋 Description

• Coordinate rehabilitation services addressing medical, psychosocial, vocational, and well-being
• Develop individualized rehabilitation plans with participants, families, and case managers.
• Serve as service integrator across medical, psychosocial, and vocational rehab services.
• Encourage participants to explore community-based interventions and resources.
• Identify barriers and implement strategies to improve outcomes.
• Coordinate communications among participants, families, providers, and stakeholders.

🎯 Requirements

• Bachelor’s degree in Psychology, Counselling, Rehabilitation, Social Sciences, Health Services
• A recognized health professional designation or CVRP/CVRP-F credential, or Quebec registration as
• At least 3 years of direct experience providing rehab services or related areas.
• Experience supporting individuals with health conditions creating participation barriers.
• Strong ability to develop plans, assess needs, and coordinate multidisciplinary services.
• Excellent communication, relationship-building, problem-solving, and advocacy skills.
