Remote Care Coordinator (Mountain Time)

📋 Description

• Coordinate care for patients with kidney disease via an integrated technology platform.
• Provide exceptional patient service and coordination across disciplines with an interdisciplinary
• Lead comprehensive care coordination focusing on Social Determinants of Health.
• Assist patients in navigating the healthcare system and accessing resources.
• Collaborate with Nurse Practitioners, Nurse Care Managers, Dietitians, Pharmacists, social workers
• Support cross-coverage between markets as needed.

🎯 Requirements

• 3 years of experience in health care coordination, community health work, or similar field
• Experience with a team-based approach to care
• Excellent knowledge of community resources
• Strong prioritization and multi-tasking abilities
• Self-directed, professional problem-solving mindset
• Strong communication and teaching skills
