Manage an assigned patient panel and address each patient’s needs within value-based care.
Collaborate with physicians, APPs, and IDT members on care plans and program goals.
Support patients in programs like Chronic Complex Care, Transitions of Care, CKD Management.
Work remotely to support Mountain Time Zone patients; hours 8:30a-5p MT.
Engage in education, transitions of care, and care plan optimization with the care team.

🎯 Requirements

Associate degree in nursing
Current RN license; compact license preferred
Care management experience required
MS Office (Outlook/Teams) familiarity
Ability to work in a remote environment with dedicated space and reliable home internet
