Remote Pharmacy Care Coordinator, MA (Contract to Perm)

📋 Description

• Telephonic patient engagement for Evergreen’s pharmacy program
• Enroll patients and explain pharmacy services over the phone
• Document consent and enrollment in the EHR
• Coordinate with care team and update patient profiles
• Manage high-volume outbound outreach and inbound support
• Support remote operations across chronic kidney disease care

🎯 Requirements

• Current CMA/RMA certification (preferred) or equivalent
• Experience in telephonic, outbound patient outreach or call-center
• Strong communication to build trust over the phone
• Comfort with EHR updates and electronic prescriptions
• Ability to manage outreach queues and follow-ups
• HIPAA privacy knowledge and secure handling of PHI
