  • Serve as a trusted partner to nephrology and primary or specialty practices on HEDIS and Stars
  • Share and interpret quality reports and dashboards to drive practical improvement steps.
  • Educate providers on documentation, coding, and workflows impacting HEDIS/Stars.
  • Facilitate recurring meetings to review trends, remove barriers, and co-develop action plans.
  • Support Pay-for-Performance components and clarify incentives and specs.
  • Coordinate cross-functional efforts to close quality gaps in care delivery.

๐ŸŽฏ Requirements

  • 1-2 years in quality improvement, population health, provider engagement, or care coordination.
  • Patient-facing outreach experience.
  • Understanding of quality workflows, gap closure, and documentation requirements.
  • Strong communication and relationship-building skills.
  • EMR proficiency and familiarity with healthcare tech tools.
  • MS Office (Outlook/Teams) intermediate skills.
