RN Case Manager - Remote

📋 Description

• Build relationships with patients and families to understand goals.
• Create personalized care plans addressing social determinants.
• Coordinate care across providers to ensure safety and continuity.
• Educate and empower members to navigate the healthcare system.
• Collaborate with an interdisciplinary team for whole-person health.
• Occasional travel up to 10% may be required.

🎯 Requirements

• Minimum of 5 years as a Registered Nurse
• Active Compact RN License
• Strong background in care management and chronic conditions
• Skilled in motivational interviewing, patient education, and patient advocacy
• Ability to work independently in a remote environment while staying connected to the team
• Comfort with technology and multiple EMR systems
