📋 Description

• Conduct in-home clinical assessments and help patients and caregivers begin services.
• Develop, implement, and update individualized plans of care.
• Build relationships with patients and caregivers via home visits and follow-up.
• Identify changes in condition, safety concerns, and barriers to care.
• Coordinate care with physicians, caregivers, internal teams, and resources.
• Educate and coach patients on care plans, best practices, safety.

🎯 Requirements

• Graduate of an accredited nursing program.
• Active and unrestricted Registered Nurse license in the State of Colorado.
• Strong clinical assessment, communication, and documentation skills.
• Compassionate, patient-centered approach to care.
• Ability to travel by car within an assigned geographic area.
• Valid driver's license and active auto insurance.
