Social Work Assistant

📋 Description

• Establish rapport with participants and families for psychosocial support
• Deliver and document social work interventions; arrange resources and care transitions
• Coordinate IDT services for housing, DMV needs, utilities, and community aid
• Complete IDT care plan reviews with participants and families within regulatory timeframes
• Conduct home, hospital, or skilled nursing visits as needed
• Assist with grievances, appeals, incident reports, and other compliance tasks

🎯 Requirements

• High school diploma or equivalency
• Associate degree in social work, human services, social science, or related field preferred
• Reliable means of transportation
• Experience assisting people with behavioral health and substance abuse issues preferred
• Experience facilitating solutions with difficult psychosocial/family circumstances preferred
• Must obtain CPR and First Aid within 180 days of hire and maintain certification
• CPR and First Aid certification upon hire preferred
